About Observation Forms

Applicants applying to the following programs must provide proof of observation in proessional settings.   The following printable forms are for verification of hours.  Please print the form and take it with you to each of your observation sites

You .may submit your application to LLU before your hours are complete.  Completion and verification of hours are required before your application is complete and an admissions decision can be made.

Please note:  The following programs fill quickly due to their popularity.  Applicants are encouraged to gain experience by exceeding the minimum hours required.

Department of Physical Therapy

Physical Therapy observation form for DPT and PTA Programs (PDF)   Minimum of 80 hours.  At least 20 hours must occur at an inpatient facility, such as a hospital or skilled nursing facility.

Occupational Therapy

Occupational Therapy observation verfication form, (PDF) 40 hours minimum -This form is to be completed by the supervising therapist 

Student reaction to OT experiences & observation, (PDF) This form is to be completed by the applicant

Radiation Technology
