Requirements for the predoctoral clerkship are as follows:

1. Completion of the junior year of dental school
2. Completion of Part I of the National Board of Dental Examiners with composite scores greater than 85 percent
3. Accumulative college grade point average of at least 3.0 out of 4.0
4. CV
5. Letter of intent (should include the dates you want to come out to Loma Linda)
6. One letter of recommendation (preferably from the OMS chair or program director who knows of your interest in OMS)
7. Letter stating your school will provide malpractice coverage
8. Copy of health insurance card
9. Copy of H1N1 Influenza and Flu Vaccine
10. Clearance for TB skin test
