DEFINITIONS
INTERNET:
Internet is the global collective of computer networks, available to millions of users world wide.
WORLD WIDE WEB:
Within the Internet is a special network of linked documents known as the World Wide Web, also known as the Web, WWW, or W3. It uses a common computer language as a publishing standard. The Web allows one to combine text, photographs, audio, and video into electronic "pages."
LLUMC PUBLIC PAGES:
Internet documents prepared and/or approved and maintained by the Loma Linda University Medical Center Office of the Vice President Public Affairs and Marketing which can be read both by the external public and in-house.
LLUMC CAMPUS PAGES:
Unofficial Internet documents which are restricted to campus (password) access only, prepared and maintained by designee originating administrator, department, program, or center.
INTERNET RESOURCES:
Documents on the Internet other than Loma Linda University Medical Center Public Pages and Campus Pages.
GENERAL GUIDELINES
1. Loma Linda University Medical Center is considered the owner of all data on LLUMC Public Pages and LLUMC Campus Pages.
2. Audiences for LLUMC Public Pages include prospective and current patients and their families, professiona1 colleagues, church leaders and members, government leaders, the media, historians, other educational institutions, other medical centers, etc. Use for other purposes is not acceptable.
3. All material(s) placed on Internet LLUMC Campus Pages must include a standard Campus Pages masthead (supplied by the Campus Web Editor) at the top of the page and, at the bottom of the page, a link to the LLUMC Public Pages, and a disclaimer as follows:
"Notice of Disclaimer: Please note that the information at this Web site is not the LLUMC homepage, and does not represent official statements or views of Loma Linda University Medical Center."
4. Information appearing on Public Pages or Campus Pages must be current and correct in grammar, content, and organization.
5. Resources should always be conserved, e.g., disk and tape storage, computer time, memory, paper, etc.
6. The Office of Campus Network Administration and/or local system administrator will keep a log of networking, inter-user communications, log-on attempts, links, and other activities. These logs will reveal usage trends and verify complaints. Backups of disk files are also made, mainly to protect users from accidents, but also to trace an abuse long after it occurs.
USE OF THE WEB
1. There are two options for placement of Loma Linda University Medical Center pages on the World Wide Web. They are:
1.1 Public Pages, which provide a means to publish much of the Medical Center's official, institutional information for the benefit of both the Medical Center and the public. Public Pages can be read both on-campus and by the public at large. The Office for Public Affairs and Marketing is responsible for preparing, placing, and maintaining all material on these pages.
1.2 Campus Pages, which are unofficial campus pages limited to password access are for LLUMC information. Eligible for a home page and its associated link to pages are: each LLUMC administrator, each department, and each recognized program and center.
2. Centers, departments, programs, associations, individuals or other groups must be authorized in writing by the Office of the Vice President for Public Affairs and Marketing to place information about their areas or the Medical Center, including telephone numbers, with other providers.
RESPONSIBILITIES FOR PUBLIC PAGES
1. Since information placed on Public Pages (including organization, content, design, and maintenance) will be viewed by the public as official, all Public Pages will be prepared under the auspices of the Vice President for Public Affairs and Marketing, who will act as official spokesperson for LLUMC.
2. Medical Center departments and individuals desiring additional material pertaining to their area to be added to the Public Pages may initiate a request for inclusion, which requires:
2.1 Submission by Administration:
a. signed approval by President, Executive Vice President, or Vice President Public Affairs and Marketing
b. completed items(s) given to the Web Editor in the Office for Public Affairs on disk for formatting.2.2 Submission by departments, staff:
a. signed approval by appropriate vice president
b. approval by Web Editor in the Office for Public Affairs and Marketing.
c. completed item placed on the Campus Pages, formatted in accordance with Public Pages layout.
3. Guidelines for acceptable content and layout for the Public Pages are placed on LLUMC Campus Pages within the Office for Public Affairs and Marketing home page.
4. A written appeal of a decision made by the Office of Public Affairs will be heard by the Medical Center Administrative Council. The Medical Center President is final authority on questions of acceptable use of the Internet. Until an issue is resolved, questionable use should be considered "not acceptable."
RESPONSIBILITIES FOR CAMPUS PAGES
1. Responsibility for all materials appearing on LLUMC Campus Pages--including any additional unit policy, organization, preparation, adherence to LLUMC policies, placement and maintenance--lies with the official designee of each unit eligible for a home page. This designee is known as the Unit Web Editor.
2. Each Unit Web Editor must be registered with the LLUMC Web Editor and the Office of Campus Network Administration, including the designee's name, title, office telephone number, pager number (if available), and authorizing signature from ranking administrator of that unit. Each Unit Web Editor carries the responsibility of compliance with Medical Center policies. Campus Pages not in compliance can be removed by the LLUMC Web Editor or the Office of Campus Network Administration without prior notice. The LLUMC Web Editor or the Office of Campus Network Administration must have the necessary access privileges to all LLUMC Campus Pages.
3. The LLUMC Web Editor will maintain on the Campus Pages a basic linking directory of eligible home pages. Each Unit Web Editor will be responsible for maintaining a detailed linking directory of that unit's pages, accessible from the unit's home page.
4. Disputes involving Campus Pages are brought before the Medical Center Administrative Council. The Medical Center President is the final authority.
SECURITY
1. The Office of Campus Network Administration shall have responsibility for maintaining the security of the Public and Campus Pages.
2. The World Wide Web Public Pages shall be set up to permit read-only access on campus, except for the LLUMC Web Editor and the Office of Campus Network Administration, whereby files can be read-- but not written to--by non-authorized LLUMC individuals; and the public cannot link to LLUMC computers beyond this point.
3. A firewall shall also be placed between the LLUMC internal systems, the LLUMC Public Pages, and public access--whereby files can be read, but not written to, by the public.
4. Authorized LLUMC Internet administrators may access user files or suspend services on LLUMC Public Pages or Campus Pages, without notice, as required to protect the integrity of computer systems or to examine accounts that are suspected of unauthorized use, misuse, or have been corrupted or damaged.

OPERATING POLICY
1. The Internet shall be made available for those who have job-related uses.
2. Initiation of service and level of access shall require an approval process which includes the following:
2.1 written request with data to demonstrate need and signature of department head.
2.2 approval of Vice President
2.3 approval of Chief Information Officer
3. Use of the Internet shall be limited to work-related purposes, to include the following:
3.1 communications or activities in direct support of patient care, advancing medical science, research, education and day-today operations.
3.2 Medical Center/University association, professional society, government or standards activities related to health care, research, and instruction.
3.3 communication and exchange for professional development, to maintain currency, or debate issues in a field or subfield of knowledge.
3.4 application for or administration of grants or contracts for research or instruction.
3.5 announcements of new Medical Center products or services for use in health care research or instruction.
3.6 communication incidental to otherwise acceptable use, except for illegal or specifically unacceptable use.
4. Unacceptable uses of the Internet shall include:
4.1 access through personally owned software.
4.2 use that distracts from performance of assigned duties, such as "surfing the net."
4.3 use for private or personal business/correspondence or providing access for a non-authorized person.
4.4 any activity which is not congruent with the philosophy of the Seventh-day Adventist Church, e.g., use of LLUMC systems or the Internet to view, store, or transfer pornographic, erotic, indecent, or other offensive materials, etc.
4.5 use of LLUMC systems for political purposes in violation of Operating Policy A-23, "Political Activities," e.g., using electronic mail to circulate advertising for political candidates.
4.6 use of the Medical Center's name to suggest or imply the endorsement of other organizations, their products or services.
4.7 soliciting or fundraising activities, except with prior approval from officially recognized advancement personnel.
4.8 disclosure of confidential patient information in violation of Operating Policy A-10, "Confidentiality of Patient Information."
4.9 harassment, libeling or slandering of other users.
4.10 any activity which violates any LLUMC policy.
4.11 destruction of or damage to equipment, software, or data belonging to the Medical Center or other users.
4.12 disruption or unauthorized monitoring of electronic communications.
4.13 unauthorized copying of copyright-protected material.
4.14 violation of computer system security.
4.15 unauthorized use of computer accounts, access codes, or network identification numbers assigned to others (see Operating Policy A-30, "Computer Access Codes and Passwords").
4.16 violation of software license agreements (see Operating Policy A34, "Unauthorized Duplication of Software").
4.17 violation of another user's privacy.
5. Information representing LLUMC or any related entity shall be placed on the Internet only with the approval of:
5.1 Vice President for Public Affairs for promotional information.
5.2 Senior Vice President for Medical Administration for medical issues.
5.3 Executive Vice President for Operations for all other issues.
6. All placements of LLUMC pages on the World Wide Web must follow the procedures stated in the attached Internet Pages guidelines, A-43.A.
7. A terminal with Internet access shall be available in the Medical Library.
8. Usage of the Internet shall be monitored to ensure authorized use.
9. Alleged violation of the terms of this policy shall be reviewed on a case by case basis.
10. Violators of this policy shall be subject to normal disciplinary procedures of the Medical Center and loss of computing privileges or Internet access may result. Illegal acts may al be subject to prosecution by the proper authorities.
10.1 Violations of a legal or ethical nature shall result in further remedial actions as indicated.
10.2 Illegal acts shall also be subject to prosecution by the proper authorities.
10.3 The Medical Center may require restitution for any unauthorized use of the Internet services.
