HIPAA

A patient has the right to view his or her records if they so choose. This does not necessarily mean we have to do it right them but we must make an appointment for them to come in. The records request must be logged in the chart.

If the patient requests that a change be made in their record, we must document in the chart: 1) the change requested, and 2) why we can't change it; or we must change the requested information and also document what was changed and send the patient a written note informing them of the change.
