Minimum expectation form requirements for certification

All clinical procedures in the practice of periodontics are identified by a specific code number. Students in the advanced education program in periodontics and implant surgery are required to acquire skill and knowledge necessary to accomplish these procedures to a level of proficiency i.e. the standard of care assigned to the periodontal specialist (see grade level 3 under grading system). Upon achieving this level of performance, the student is required to perform a limited number of the following procedures using a special data collection form. The student will define this specific clinical objectives to be achieved as an outcome to the treatment procedure. The objectives are recorded from a fixed landmark and are archived. Six months after the clinical therapy is rendered, the patient is re-evaluated using the same landmark to establish the outcome. The outcome is then compared to the "expectation" of outcome recorded six months prior, and the results are a source of learning and discussion during our clinical conference seminars. The following is a list of minimum procedures required for "expectation" form documentation.
