All new students are REQUIRED to have the immunizations listed below completed before their first registration. Students will not be allowed to register without a valid and completed immunization record.
We strongly recommend that these immunizations be obtained BEFORE arrival on campus. All of these immunizations, except the third Hepatitis B, can be completed in one month. Many county health departments offer these immunizations and may be up to 20 percent less expensive than the Student Health Service prices.
An LLU Pre-Entrance Health Requirement form (PDF) must be sent to Student Health Service for verification of immunization.

I. IMMUNIZATIONS:
1) Measles, Mumps, Rubella (MMR) -- Series of 2 injections, must be current after 1980 or a positive MMR titer.
2) Tetanus/Diphtheria -- Must be current within 10 years. Clinical programs my require Tdap.
3) Hepatitis B -- Series of 3 injections, Recombinant form-Engerix-B.
4) Tuberculosis Skin Test (PPD Mantoux) -- Must be current within 6 months. (If you test positive, please bring a chest X-ray REPORT done within the last year.)
5) Varicella (chickenpox) -- Have you had chickenpox in the past? If NOT then you must show proof of either a series of two injections OR a positive Varicella titer.
II. ISHIHARA'S COLOR BLINDNESS TEST -- Required ONLY for medical technology and cytotechnology programs.
III. VISUAL ACUITY VERIFICATION -- Required ONLY for School of Dentistry (Please send this verification directly to the School of Dentistry.)
IV. PHYSICAL EXAM -- Refer to your program for specific requirements.
REMEMBER: A valid and completed immunization record is required before you can register.
