Chairperson facilitated production of data regarding past and current electives offered at the School with recommendations for improvement, revision, and removal of elective courses to both departments within the School. School of Pharmacy Executive Council-Faculty Representative Executive Council is a standing committee at the Loma Linda University School of Pharmacy. Council is composed of the Dean, Associate Deans, Department Chairs, and Financial Officer of the School. Faculty representative to Executive Council is voted by the faculty of the School to serve as a liaison and representative of the concerns and needs of the Faculty at the School of Pharmacy. Faculty Representative presents issues as dictated by the faculty and reports back to faculty on any items or resoluations discussed in the Council. ( 7/2008 - 6/2009 )

Curriculum Committee Past-Chair serves the final year as a chair to the Committee, as per the by-laws of the Committee. Past-Chair is responsible for assisting the current Chair with syllabi review process, course review process, curriculum oversight and implementation, and oversight of new and elective courses, as well as well as assignment appropriate tasks to Curriculum Committee members. Past-Chair assists the Chair with maintaining record of actions performed by the Committee. School of Pharmacy Accreditation Oversight Committee-Member Accreditation Oversight Committee was charged with responsibilities and tasks related to the School of Pharmacy accreditation process with the American Council for Pharmaceutical Education (ACPE) accreditation. AOC members were composed of Assessment Committee Chair, Curriculum Committee Chair, and administrative faculty who were appointed to facilitate and complete all evaluations and assessments of ACPE Standards that LLUSP was responsible for addressing to achieve full accreditation as a school of pharmacy. As a member of AOC, specific standards included assessing and evaluation the student perception of over-reliance of multiple choice questions in graded activities. ( 7/2008 - 1/2009 )

Educational Effectiveness Committee Committee Member Educational Effectiveness Committee is a standing committee at Loma Linda University. Membership is appointed from the Office of the Chancellor of Loma Linda University. Members of the Committee represent the various schools at Loma Linda University such as dentistry, medicine, nursing, pharmacy, public health, graduate studies, religion, and science and technology. The Curriculum Committee Chair-Elect participated in regular meetings in the Curriculum Committee. Curriculum Committee Chair-Elect served as a formal liaison to the Assessment Committee to communicate any issues or collaborations necessitated by either committee. Chair-elect served as a Chair-in-training prior to serving as full Chair upon the departure of the current Chairperson. Chair-Elect assisted in the oversight of the Curriculum Committee including responsibilities as syllabus review, development of policies and procedures, committee bylaws, or facilitating Committee meetings. Chair-Elect also reviews and evaluates all didactic course syllabi for the School of Pharmacy academic curriculum, as well as provide recommendations and oversight over the School?s didactic courses, electives, schedules, and course policies. LLU School of Pharmacy Admissions Committee - As a member of the LLU School of Pharmacy Admissions Committee, Dr. Hernandez participated in attending Committee meetings, applicant file and transcript review, applicant interviews, and provided interview form revisions. According to Admission Committee Records, Dr. Hernandez participated in 23 personal interviews counting for 14.67% of the 150 candidates who interviewed at the School. This interview process occurred between November 2005-May 2006. ( 8/2005 - 12/2006 )

Sub-Committee to Academic Standing and Professionalism Committee-Student Grievance - Dr. Hernandez served as a member of the Sub-Committee to the Academic Standing and Professionalism Committee in July 2006 to deliberate and provide recommendations to the Academic Standing and Professionalism Committee regarding a student grievance, which may be related to student progression through the program. As a member of an impartial panel, appropriate recommendations were made based on case facts. Recommendations were made to the Academic Standing and Professionalism Committee at the conclusion of all meetings. ( 7/2006 )

Accreditation Council for Pharmacy Education (ACPE) Self-Study 2005-2006 - Dr. Hernandez was appointed to two Accreditation Council for Pharmacy Education (ACPE) Self-study Committees (#3-The Curriculum in Pharmacy and #4-Standards for Students). In the ACPE #3-Standards for Curriculum in Pharmacy, Dr. Hernandez participated in preparing documentation for addressing a total of 6 guidelines. Five of the six Guidelines pertained to Guileline #3-Standards for Curriculum, such as #8-The curriculum in pharmacy, #9-Curricular organization and length, #12-Teaching and learning processes, #13-Evaluation of student achievement, #14-Curriculum evaluation. As part of the committee to prepare the documentation for Guideline #4-Standards for Students, Dr. Hernandez participated in preparing documents for guideline #18-Progression of Students. Sub-committee to Academic Standing and Professionalism-Recommendations to the "Bridge Course" - Dr. Hernandez served as a faculty participant to the Sub-committee to Academic Standing and Professionalism Committee for providing recommendations for the "Bridge Course" for students who had failed a class in the School of Pharmacy curriculum. Duties included meeting the Assistant Vice Chancellor and various faculty from the School to develop and revise critical components to the Bridge Course. Results and recommendations made by the Sub-committee were submitted to the Faculty at the School for review and approval/disapproval. ( 1/2006 - 2/2006 )

Faculty of Graduate Studies Working Group: New and Modified Programs - Dr. Hernandez was appointed to the Faculty of Graduate Studies (FGS) Working Group for New and Modified Programs. This working group was appointed to revise the procedures for "revising or approving new programs in the FGS". During the course of the working group's discussions, various procedures relative to specific language to be included in the standard operating procedures were discussed, revised, and approved. Course Evaluation/Instructor Evaluation Form Revision - Dr. Hernandez along with Dr. Davis and Dr. Mosavin, revised the old instructor/course evaluations forms to reflect more pertinent questions and survey items that would provide the most efficient reflection of course instruction and content delivery. The evaluation forms now include a matched response format in which anonymous written responses can now be tracked to the quantitative responses given by the corresponding anonymous respondent. Dr. Hernandez is trained in both quantitative and qualitative research design and methodology. His expertise was used to develop the questions and evaluation format for the revisions. Ad Hoc Committee to the Promotion Tenure Committee for Faculty Peer Review - Dr. Hernandez participates in the ongoing development of a faculty peer review process that is targetted towards evaluation of faculty teaching by faculty peers at the School of Pharmacy. Activities include developing evaluation tools such as lecture evaluation worksheets, pilot testing evaluation instruments in classes taught by members of the Ad Hoc committee, as well as participate in regular meetings to discuss and debrief the development process. Processes, tools, and procedures will be presented to the faculty at the School after thorough development and discussion of the processes and tools are completed. ( 2/2007 - Present )

Accreditation Council for Pharmacy Education (ACPE) Self-Study 2006-2007 - Dr. Hernandez participated in the working group to provide a progress report and update on the curricular changes and ?dry-labing? to the School of Pharmacy?s educational curriculum. Dr. Hernandez collaborated with additional faculty members to report on these changes. A written report was created and submitted to the Steering Committee for Accreditation at the School of Pharmacy. This written report was to provide information to the ACPE report which is submitted to ACPE for the School of Pharmacy?s self-study for accreditation. ( 11/2006 - Present )

LLU School of Pharmacy Curriculum Committee - As a member of the Curriculum Committee at the School, Dr. Hernandez participated as a faculty voting member in the Committee. Faculty writer reports to the Curricular Standards Team Leader and the Associate Dean for Academic Affairs in all aspects of data analysis, manuscript writing, and document preparation for the accreditation document. Dr. Hernandez made substantive writing contributions to standards 9-13.
